Patient Forms
All new patients need to fill out forms prior to the first appointment. In order to save time, we have provided links to our forms. Please feel free to fill out the forms and bring them with you to your appointment, or they may be faxed ahead of time to 585-301-4037.
Please bring a photo identification and your insurance card with you to all appointments. For established patients, please notify the receptionist upon arrival to our office of any changes in your insurance and/or contact information.
